Purpose: This form is used by GroupX Employee to request reimbursement for all pre-approved proprietary format licenses and certifications s/he is authorized/requested to use at California Family Fitness in the leading of approved formats/classes("activities"). Receive approval from the GX Department head prior to purchase of proprietary format license/certification.
Instructions: While clocked in, complete this form. Provide all receipts, class details, etc. Completed forms must be forwarded no later than the last day of the month in which approved proprietary format certification(s) or license(s) was/were purchased. Late forms will result in discipline which may include separation from employment.
